By submitting the Little Sequoits Preschool Enrollment Form you are agreeing to the following.
Please arrive on time for pick up. You have a 5-minute grace time, however, if you are late the following procedure is in place: The 1st time you are late, it is excused, the 2nd time you are late, you will receive a warning, the 3rd time you are late, your child could be dropped from the program.
I hereby certify that my child is currently covered by a health and accident insurance policy and should an accident occur during the preschool program that he/she is enrolled in, that said policy will cover resultant expenses and medical consultation. I consent to the enrollment of my child in the Little Sequoits Preschool Program and agree that Antioch Community High School as well as director Mrs. Megan Bernard shall not be responsible in case of sickness or injury to my child while in attendance at the program.
In case of an emergency rising out of serious illness or injury, permission is hereby granted to transport my son/daughter to a medical facility, if necessary, and to provide necessary treatment. I understand that an attempt will be made by the school administration, the sponsor of the organization, or the attending physician to contact my spouse or me in the most expeditious way possible. If said physician is not able to communicate with me or my spouse, permission is hereby granted to proceed with necessary medical or surgical treatment in the best interest of my son/daughter, and if necessary to admit him/her to a medical facility. The undersigned has read this Parent Consent Agreement, understands its terms, and executes it voluntarily.
Throughout the school year, the District may use photographs, videos, or digital images used for informational or news-related purposes (whether by a media outlet or by the school) of any student participating in the Little Sequoits Preschool Program.
The "Event Organizer" is not obligated to provide any refunds in regards to tickets purchased for their events.
Vanco Events is an online system for organizations and community groups that has been designed to look after the entire event ticketing needs.
All issues relating to the sale of tickets and the hosting of their associated event(s) are the responsibility of the “Event Organizer” and not that of Revtrak inc d/b/a Vanco Events.
